1. Service Agreement

By engaging the disposal services of Phoenix Medical Waste LLC, the customer (generator) agrees to comply with all packaging and segregation guidelines. Only regulated medical waste (RMW) as defined by Missouri state law should be placed in the provided containers.

2. Prohibited Waste

The customer agrees NOT to place hazardous chemicals, radioactive materials, or bulky pressurized cylinders in containers provided by Phoenix Medical Waste LLC. Contaminated shipments may be rejected at the generator's expense.

3. Pickup Schedules & Fees

Phoenix Medical Waste LLC operates on a pre-determined pickup frequency. Missed pickups due to facility closures or inaccessible containers may incur a trip charge. Standard payment terms are Net 15 from the date of the manifest.

4. Jurisdiction

This agreement shall be governed by the laws of the State of Missouri. Any disputes arising from services provided by Phoenix Medical Waste LLC shall be handled in the courts located in St. Louis County, MO.
